SUTTER MIDDLE SCHOOL ATHLETICS
The three steps below are required to participate in any athletic sport at Sutter Middle School.

SPORTS PHYSICAL - STEP 1
COMPLETE BEFORE FIRST TRYOUT - Participation in tryouts will not be allowed without a completed FCUSD physical THAT NEEDS TO BE UPLOADED TO THE ONLINE CLEARANCE, along with "Step 2" (below) being completed.
Coaches will NOT collect a hardcopy of Physicals from students. Coaches will receive a list of students "CLEARED" from the online clearance (via the Athletic Director).
To participate in or tryout for a school sport, all athletes must have a completed the FCUSD Sports Physical Form (top half filled out by a parent or guardian and signed/dated; bottom half completed by a physician, physician assistant, or nurse practitioner and signed. Include the medical office stamp near the signature. Please upload it to:
The Sports Physical Form is good for one calendar year and must be dated on or after May 31 of the prior school year. Please note, chiropractors and physical therapists cannot sign an athlete’s Sports Physical Form.We unfortunately do NOT accept Bulldog Football Physicals. We only accept the FCUSD Physical form. If you already have an alternative physical form, PLEASE bring the FCUSD physical form to your health care provider for them to fill out and sign/date accordingly.

TRANSPORTATION - STEP 3
PARENT TRANSPORTATION
Parents will be responsible for transporting their students to and from all games/meets, therefore a Personal Auto Use Form & Student Transportation Form are required and must be completed. These 2 forms will be handed out by the coach at their 1st team practice and is mandatory if you'd like your child to attend away games (or anything associated with their team off campus.)
An Early Release Form will also be given to athletes that must be signed by parents and turned in, which will allow athletes to go straight to their parents' car when given early release for games/meets. If the early release form is not turned in, parents will be required to come in to the office and sign their athlete out for every game/meet that requires early release.
TRANSPORTATION FEE AND DONATION
There will be no transportation fee this season due to a bus driver shortage. Please consider donating to our sports programs. Donations can be submitted to our account clerk in the office or processed online. Transportation Fee and Donation Form
